Having a long term condition or long term needs following a severe illness or trauma can result in uncertainty about the future and reduced independence within everyday situations. Looking after yourself, managing a home, fulfilling family roles or work, education and leisure interests can become difficult or seemingly impossible.

Early access to rehabilitation is proven to reduce your likelihood of further health problems and long-term care needs.
